CDC Updates Gonorrhea Treatment Guidelines as Drug-Resistant Strains Spread

The Centers for Disease Control and Prevention has updated its gonorrhea treatment guidelines in 2026 to address a growing number of drug-resistant infections, as the World Health Organization documents a sixfold global rise in azithromycin-resistant gonorrhea, according to a review published by Medical Daily.
Roughly 1.6 million gonorrhea cases are reported annually in the United States. Ceftriaxone, an injectable cephalosporin, has been the last broadly reliable first-line treatment after the bacterium developed resistance to sulfanilamides, penicillins, tetracyclines, and fluoroquinolones over the past eight decades, according to the American Sexual Health Association.
Reduced-susceptibility and fully ceftriaxone-resistant strains have now been documented in U.S. cities. The updated 2026 CDC guidance specifically addresses how clinicians should manage suspected treatment failures and confirmed resistant infections, with gentamicin combined with azithromycin identified as a remaining option when ceftriaxone fails, according to the Medical Daily review.
Genomic surveillance adds further detail to the picture. Minnesota health officials reported that disseminated gonococcal infections — cases where the infection spreads beyond the initial exposure site — increased nearly fourfold in 2024, prompting expanded whole-genome sequencing of isolates. That surveillance identified a new strain, designated ST16676, which became the dominant strain among the state's disseminated infection cases collected between June and September 2025, replacing a previously dominant strain.
Public health researchers say the core challenge is that Neisseria gonorrhoeae is unusually adept at acquiring resistance mutations, meaning each new first-line antibiotic introduced since the 1930s has eventually lost reliability.
Because standard antibiotics remain effective against most cases for now, testing promptly and completing any prescribed treatment fully remains the most direct way to avoid contributing to resistance.
