Minnesota Genomic Surveillance Tracks New Gonorrhea Strain Behind Rising Disseminated Infections

A near-fourfold increase in disseminated gonococcal infections in Minnesota prompted state health officials to expand genomic surveillance of Neisseria gonorrhoeae, leading to the identification of a newly dominant strain circulating in the state.
Disseminated gonococcal infection (DGI) occurs when gonorrhea spreads from the original site of infection to other parts of the body, including joints and the bloodstream. After DGI cases rose sharply in 2024, the Minnesota Department of Health began whole-genome sequencing of isolates to track the outbreak's genetic makeup, according to a study posted to medRxiv.
The sequencing identified a strain designated ST16676, which did not match any previously documented antimicrobial-resistance profile in the national reference database, according to the study. Of 22 sequenced isolates collected between June and September 2025, roughly 64% belonged to this new strain, which replaced a previously predominant strain identified in 2024.
Notably, researchers found the new strain carried the same genetic marker previously associated with a higher risk of disseminated infection as the strain it replaced, suggesting the outbreak's underlying risk profile has persisted even as the dominant genetic lineage shifted.
Minnesota's health department continues to track syphilis, gonorrhea, and related antibiotic-susceptibility data on an ongoing basis, according to the agency's public STI statistics page, which is updated as new case reports come in.
